  • Application checklist below (7 parts):

MUST BE SUBMITTED ELECTRONICALLY to applications@chicagoscholars.org  (MAKE SURE SUBJECT OF YOUR EMAIL CONTAINS "APPLICATION 2009"). 
  1. Application Form icon 2009 SCHOLARSHIP CHICAGO APPLICATION
  2. Three short answer essays (each answer should be between 100 and 300 words) in MS Word format only with your LAST_FIRST name as a filename.
  3. Recent portrait style photo in JPG or JPEG format only with your LAST_FIRST name as a filename.
CAN BE SUBMITTED VIA REGULAR MAIL to our office
  1. Signature page of the application.
  2. High school transcript (in a sealed envelope)
  3. One (1) Teacher Recommendation icon TEACHER RECOMMENDATION FORM
  4. One (1) Counselor Recommendation icon COUNSELOR RECOMMENDATION FORM

ESSAY QUESTIONS:

  1. Write an essay about what you value most in life. Select or create a picture that best illustrates your essay. Your picture may be UP TO 8 ½'' x 11" but not any larger. All photos become the property of Scholarship Chicago and will not be returned.
  2. We are seeking scholars who are commited to our program. If you are selected to participate in Scholarship Chicago, please indicate which areas you believe you would receive the maximum value.
